Caregiver Simi Valley overnight shifts 7pm to 6 am

Introduction:

Family Ties Home Care has a client in Simi Valley who needs night time help so his wife can get her sleep. Client is in his 70's with dementia and is a fall risk. He needs to be watched and assisted when he gets up at night to use the restroom. This a very active night shift and client needs help getting up and down often.

Responsibilities:

Following a pre-set care plan
Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance)
Assist with medication reminders
Monitor and report on client's well-being
Light housekeeping, laundry, & other household daily chores as needed
Grocery shopping & errands
Cooking/meal preparation
Scheduling appointments, including doctor's
Communication with Family Ties HC management & family members/loved-ones regarding the client's daily status
Daily notes
Other duties assigned as needed

Qualifications:

Current HCA or higher certification
Caregiving experience of 1 year or similar experience
Compassionate and caring demeanor
Excellent communication & interpersonal skills
Ability to follow the pre-set care plan and medication schedule
Ability to lift 35 pounds
Ability to assist with client's mobility as needed
Valid driver's license and reliable transportation
Ability to work the schedule as described

Caregiver Westlake Village several days a week available 9:30-8:00 PM

Family Ties Home Care has an opening for a client in Westlake Village who has advanced dementia and needs daily help. Lives with her husband, she is physically fine but has major cognitive issues. Very active and social loves to walk, sing, play games and do puzzles. This is a very hands on and active shift. She can be a fall risk as well.

Introduction:

Family Ties Home Care has an opening for a client in Malibu who is need of help with daily ADL's. She is very independent but is recovering from a broken shoulder so has very little use of her right hand and is also starting treatment for Leukemia. She needs stand assist, meal prep and someone to drive her to appointments . Very sweet woman but loves her independence and is learning to accept help, she is very overwhelmed and needs a gentle kind CG to help her navigate this time in her life

North Hollywood: Family Ties Home Care has an opening for a senior lady who needs assistance with All ADLs and SAFETY. HOYER LIFT EXPERIENCE A MUST. Also help with hygiene, meals, med reminders, light housekeeping, and household chores, etc..

Must have Caregiving experience, a Driver's license, and a Car.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

Following a pre-set Care Plan
Compassionate companionship
Helping patient with daily functions, such as dressing or bathing
Light housekeeping, such as vacuuming or laundry
Grocery shopping and cooking meals
Assisting patients with medication reminders
Scheduling doctors appointments or other meetings and organizing transportation
Recording and reporting to the supervisor about the patient's status
Teaching patient self-care skills
Other duties as assigned

Minimum Qualifications (Knowledge, Skills, and Abilities)

Current and valid HCA/HHA certification
Ability to lift a minimum of 35 pounds
Strong patient service skills
Attention to detail and ability to carefully follow protocols and rules regarding the patient's care
Ability to address minor medical issues
Availability to work evenings, holidays, and weekends
Experience with housekeeping tasks, including keeping the house clean and safe for the patient to live in and doing the dishes
